Does running help with weight loss?
Running by itself produces only modest weight loss. A 2011 systematic review by Thorogood and colleagues in the American Journal of Medicine found isolated aerobic exercise programs of six to twelve months produced a weighted mean difference of -1.6 to -1.7 kg against no-exercise controls, in adults with overweight or obesity. A 1997 meta-analysis by Miller, Koceja and Hamilton found short-term weight loss in obese adults from exercise alone (2.9 kg) was smaller than from diet alone (10.7 kg) or diet plus exercise (11.0 kg), and a 2009 meta-analysis by Wu and colleagues, in adults with overweight or obesity, found adding exercise to diet produced a modest, statistically significant long-term edge over diet alone.
How many calories running burns per mile
The American College of Sports Medicine's running oxygen-cost equation, published in ACSM's Guidelines for Exercise Testing and Prescription, estimates the oxygen a runner uses per minute (VO2, in mL per kg of body weight) as 3.5 plus 0.2 times running speed in meters per minute. Topend Sports prints the same equation as VO2 = 3.5 + 12v + 54gv, with v the speed in metres per second and g the gradient; on level ground, 12 times a speed in metres per second is the same number as 0.2 times that speed in metres per minute. As reproduced by Topend Sports, ACSM states the equation is validated for steady-state running at roughly 3.1 to 15.5 mph (5 to 25 km/h). Topend Sports' reproduction of the equation states accuracy is highest, within about 3 to 5% for trained athletes running at a steady pace, over a narrower 3.1 to 12.4 mph (5 to 20 km/h) band.
ACSM publishes the oxygen-cost equation; the per-mile figure below is this page's own calculation from it. Converting the oxygen cost to calories with the standard estimate of about 5 kcal per liter of oxygen works out to roughly 1.7 to 1.8 kcal per kilogram of body weight (about 0.77 to 0.82 kcal per pound) for each mile run, with the exact figure depending mainly on body weight and only slightly on pace within the validated range.

What the evidence shows: exercise alone, diet alone, and combined
A 2011 systematic review and meta-analysis by Thorogood, Mottillo and colleagues in the American Journal of Medicine pooled 14 randomized controlled trials, 1,847 patients with overweight or obesity, testing aerobic exercise on its own, with no diet component. Programs lasting six months produced a weighted mean difference in weight of -1.6 kg (95% CI -1.64 to -1.56) against control groups; programs lasting twelve months produced -1.7 kg (95% CI -2.29 to -1.11). Waist circumference fell by a comparably modest amount at both durations. The review's own conclusion is direct: isolated aerobic exercise is not an effective weight-loss therapy by itself, though it produced modest benefits to blood pressure and lipids and may still work as part of a combined program with diet.
An earlier meta-analysis, by Miller, Koceja and Hamilton in 1997 in the International Journal of Obesity and Related Metabolic Disorders, reviewed more than 700 studies and accepted 493 for analysis, all of them interventions for weight reduction in obese adults (mean 92.7 kg, BMI 33.2, mean age about 40). Over a typical 15-week intervention, mean weight loss was 10.7 kg (SE 0.5) on diet alone, 2.9 kg (SE 0.4) on exercise alone, and 11.0 kg (SE 0.6) on diet plus exercise combined. At one-year follow-up, the diet-only groups had maintained 6.6 kg (SE 0.5) of that loss and the diet-plus-exercise groups had maintained 8.6 kg (SE 0.8); the meta-analysis did not report a separate one-year figure for exercise alone. The authors describe diet plus exercise as tending to be the superior program at one year, though the short-term numbers show diet doing most of the work either way.
A 2009 meta-analysis by Wu, Gao, Chen and van Dam in Obesity Reviews, restricted to 18 trials in obese or overweight adults with at least six months of follow-up, found diet-plus-exercise programs produced significantly more weight loss than diet-only programs: 1.14 kg more (95% CI 0.21 to 2.07) and 0.50 kg/m2 more reduction in BMI (95% CI 0.21 to 0.79). Both intervention types still showed partial weight regain over time, a gap the review's authors describe as needing further research rather than a single fix.
How to apply this to a running routine
Use the table above as a rough planning tool for weekly volume. Pair it with a modest, sustained calorie deficit if weight loss is the goal, since diet accounted for most of the weight lost in the meta-analyses above. Common mistakes when running for weight loss
- Expecting running alone to match a diet change. Thorogood et al.'s pooled figure was -1.6 to -1.7 kg over six to twelve months; Miller et al.'s meta-analysis put exercise-alone weight loss at 2.9 kg against diet alone's 10.7 kg over 15 weeks.
- Treating "calories per mile" as an exact number. The ACSM-based estimate above is a group-level equation, accurate to within roughly 3 to 5% for trained athletes at a steady pace; it describes a typical runner in that speed range, and running economy shifts the true number for any one person.
- Cutting calories sharply to speed things up. The CDC's guidance on losing weight frames roughly 1 to 2 pounds a week as the steadier pace to aim for.
See a doctor or dietitian if
- You don't currently exercise regularly and have a known cardiovascular, metabolic or renal condition, or symptoms suggestive of one. Riebe and colleagues' 2015 update to ACSM's exercise preparticipation screening guidelines, in Medicine & Science in Sports & Exercise, recommends medical clearance first in that situation.
- You want to lose weight faster than the roughly 1 to 2 pounds a week the CDC's guidance on losing weight describes as the steadier pace, or you take medication that weight change could affect.
- You want an actual diet plan beyond a general calorie-deficit range. That calls for a registered dietitian.
What to do next
If running is new, starting when unfit covers the on-ramp before volume or calories matter much.
