The marathon does real, measurable damage to muscle tissue, and the evidence on how long it takes to repair comes from a handful of studies that each measured something different: muscle biopsies, jump power, illness rates. None of them agree on a single number, because they weren't measuring the same thing. The table below lines up what each source found against days since the race, and what a published coach's plan schedules for that window.

What the evidence says, by days after the race

This table is Runked's own, built by placing each cited study's findings and Hal Higdon's published Novice Post-Marathon Recovery schedule against the same timeline. Where two sources measured different things in the same window, this table keeps both entries side by side.

(1990) found 12.9% of 1,828 Los Angeles Marathon finishers self-reported an infectious illness in the week after the race, versus 2.2% of similarly experienced runners who didn't race that week (odds ratio 5.9, 95% CI 1.9 to 18.8). Higdon's plan allows one short run, about 2 miles, around day 4, plus cross-training; still no hard efforts. Treat a cold or sore throat this week seriously; don't run through it.
Weeks 2-3 Takayama et al. (2017) tested 11 recreational runners and found changes in VO2max, running economy and anaerobic threshold from before the race to 7 days after small enough that the study's own analysis called them possibly trivial to unclear, with perceived soreness back to baseline by days 4 to 7. Its only post-race measurement was that day-7 test at the end of week 1; it measured nothing across weeks 2 and 3. That's a faster-looking picture than Warhol's muscle-repair timeline below; Takayama's team measured aerobic capacity and soreness in recreational runners, and it did not track the ultrastructural repair Warhol measured by biopsy in a different, larger sample. Higdon's plan builds to 2-4-2 then 3-4-3 mile midweek runs, with a 30-to-60-minute weekend long run. No speedwork yet.
Weeks 3-5+ Warhol et al. found progressive repair of the mitochondrial and myofibrillar damage by 3 to 4 weeks, and late biopsies at 8 to 12 weeks showed the central nuclei and satellite cells characteristic of a completed regenerative response. Pete Pfitzinger and Scott Douglas's Advanced Marathoning, as summarized by RunDida, gives a rule of thumb of one easy day for every mile raced, about 26 days for a marathon. Higdon's plan reaches 4-5-4 then 5-5-3 mile weeks and closes week 5 with a non-competitive 5K or half marathon. Save hard workouts and racing for after that.

What the studies actually measured

Robert Hikida and colleagues' 1983 study, published in the Journal of the Neurological Sciences, biopsied the gastrocnemius muscle of volunteer marathon runners before the race and at intervals for 7 days after, examining the tissue ultrastructurally. It found muscle fiber necrosis, inflammation, and phagocytic cell infiltration in most samples, including some pre-marathon ones, with Z-line streaming, disrupted sarcolemma and other severe changes most prevalent at 1 and 3 days post-race. Michael Warhol and colleagues' 1985 study in the American Journal of Pathology examined gastrocnemius tissue by electron microscopy from 40 male marathon finishers and 12 male nonrunners. It found focal fiber injury (edema, myofibrillar lysis, T-tubule disruption) without inflammatory infiltrate at 1 to 3 days, progressive repair by 3 to 4 weeks, and by 8 to 12 weeks, central nuclei and satellite cells, the hallmark of muscle regeneration.

Kim Petersen and colleagues' 2007 study in the European Journal of Applied Physiology tested 8 highly trained runners before a marathon and at 30 minutes, 2 days and 5 days after, measuring countermovement jump power and knee-extensor and plantar-flexor strength. Jump power was still reduced at day 5, though less than at day 2. David Nieman and colleagues' 1990 study in the Journal of Sports Medicine and Physical Fitness mailed a questionnaire to 4,926 of 12,200 Los Angeles Marathon applicants and got 2,311 responses about self-reported infectious episodes before and after the race; the study measured self-reported illness and did not isolate a specific immune mechanism, and its own confidence interval (1.9 to 18.8) is wide, reflecting how few runners in the small non-racing comparison group got sick (3 of 134). Fuminori Takayama and colleagues' 2017 study in the Journal of Sports Medicine tested 11 recreational runners 1 to 2 weeks before a marathon and again 7 days after, and found changes in aerobic fitness measures from before to 7 days after small enough that the study's own analysis called them possibly trivial to unclear, with soreness back to baseline by then too, in contrast to the slower muscle-repair and strength timelines above; a smaller, more moderate-effort field and different outcome measures likely explain the gap, and neither study is wrong.

How to apply this

Higdon's Novice Post-Marathon Recovery plan is the clearest published week-by-week schedule that lines up with the evidence above: 3 days completely off, a short easy run and cross-training in the rest of week 1, then a gradual midweek-mileage rebuild (2-4-2, then 3-4-3, then 4-5-4, then 5-5-3 miles) alongside a weekend long run that grows from 30 to 60 minutes, closing week 5 with a non-competitive 5K or half marathon at easy effort. Pfitzinger and Douglas's one-easy-day-per-mile-raced guideline offers a simpler rule of thumb, landing in the same neighborhood, about 26 days for a marathon, before targeting a hard workout or race. Either way, the first 3 days matter most: that's the window Hikida's and Warhol's biopsies flag as peak injury, and it's also where Higdon's plan is strictest.

Common mistakes

See a doctor if pain persists or worsens instead of easing over the first week, or if urine turns dark brown, red or tea-colored (a rhabdomyolysis warning sign, per the Cleveland Clinic). Call emergency services right away for chest pain or pressure during or after exercise, especially pain spreading to the arm, shoulder, neck, jaw or back, per the Cleveland Clinic's exercise chest-pain guidance. None of the above is a diagnosis.

Warhol et al.'s 1985 biopsy study found the underlying muscle repair process runs 3 to 4 weeks, with full regenerative signs only showing up 8 to 12 weeks out, so lingering weakness at day 5 fits the timeline; it doesn't signal a problem on its own.

Why am I more likely to get sick the week after a marathon?

Nieman, Johanssen, Lee and Arabatzis's 1990 study in the Journal of Sports Medicine and Physical Fitness followed Los Angeles Marathon entrants and found 12.9% of the 1,828 finishers who hadn't been sick beforehand self-reported an infectious illness in the week after the race, versus 2.2% of similarly experienced runners who didn't race that week, an odds ratio of 5.9 (95% CI 1.9 to 18.8). The study measured self-reported illness episodes. It did not isolate a specific immune mechanism, and the pattern is still consistent enough that treating the week after a marathon as a higher-risk window is reasonable.
